Indications/Uses
Infections caused by clarithromycin-sensitive pathogens: Nasopharynx eg, tonsillitis, pharyngitis, paranasal sinus & lower resp tract infections eg, bronchitis, bacterial & atypical pneumonia; skin infections eg, impetigo, erysipelas, folliculitis, furunculosis & septic wounds.
Dosage/Direction for Use
Adult ≥12 yr 250 mg bd for 7-14 days, may be increased to 500 mg bd in severe infections. Childn 7.5 mg/kg bd for 5-10 days. Treatment & prophylaxis of disseminated infection due to Mycobacterium avium complex 500 mg bd w/ other antimycobacterials. H pylori eradication associated w/ PUD 500 mg bd for 7-14 days w/ other antibacterials & PPIs or H2-receptor antagonist.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to clarithromycin, erythromycin or other macrolides. Concomitant use w/ terfenadine, astemizole, cisapride, pimozide; ergot alkaloids; domperidone.
Special Precautions
Discontinue use if severe acute hypersensitivity reactions occur. Not to be used in patients w/ history of acute porphyria. Overgrowth of nonsusceptible bacteria; superinfection. Not recommended in concomitant use w/ ranitidine, bismuth citrate in patients w/ CrCl <25 mL/min. Severe renal impairment. Pregnancy & lactation.
Adverse Reactions
GI disturbances; headache; skin rashes; transient increased SGOT & SGPT.
Drug Interactions
Increased serum conc of theophylline, carbamazepine, digoxin, terfenadine, astemizole, cisapride, pimozide. Potentiated effects of oral anticoagulants. Potentially serious toxicity w/ ergot alkaloids. QT prolongation & cardiac arrhythmias w/ domperidone. Ritonavir, other CYP3A4 inhibitors or CYP3A4-metabolized drugs.
